Request Form

Please review the Planning a Visiting Scholars Presentation page for instructions on how to request a scholar. You may download the request form below, complete it and return it to us by any of the following means:

1. Print the PDF, complete it and fax it to (302) 657-0655.
2. Print the PDF, complete it and mail it to 100 West Tenth St., STE 1009 Wilmington, DE 19801.
3. Type directly onto the PDF version. You can save it to your files and use Adobe Acrobat's typewriter or other text editing tools to complete the form online. You can also save it as an .rtf and edit it in Word, or copy and paste its contents into a Word document. Complete and send the application via email to jacross@dhf.org.
4. Submit your request through our online system. (See below.)
